DATA SUBJECT APPLICATION/REQUEST FORM REGARDING THE PROTECTION OF PERSONAL DATA

INFORMATION

Pursuant to Article 13 of the Personal Data Protection Law No. 6698 (KVKK):

The data subject submits their requests concerning the implementation of this Law to the data controller in writing or by other methods to be determined by the Board. The data controller concludes the requests in the application free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request. However, if the process requires an additional cost, the fee set out in the tariff determined by the Board may be charged. The data controller either accepts the request or rejects it, stating its reasons, and notifies the data subject of its response in writing or electronically. If the request in the application is accepted, the data controller takes the necessary action. If the application arises from an error by the data controller, the fee charged is refunded to the data subject.

Accordingly, as the data subject/applicant, you may submit your requests and applications to us, as a “data controller” under the KVKK, using this form through one of the following options:

1. The applicant applies in person at our address below, proving their identity by presenting their Republic of Türkiye Identity Card;

2. The written application must be submitted through a notary public or sent to our contact address by registered mail with return receipt.

The methods listed above qualify as “written” applications under paragraph 1 of Article 13 of the Law. Once any other methods to be determined by the Board are announced, data subjects will also be informed of how applications will be accepted through those methods.

Pursuant to paragraph 2 of Article 13 of the Law, the applications you submit to us will be answered free of charge “as soon as possible and within thirty days at the latest” from the date your request reaches us, depending on the nature of the request. However, if the process requires an additional cost, we will charge the fee set out in the tariff determined by the Personal Data Protection Board. Our responses will be delivered to you in writing or electronically in accordance with Article 13 of the Law.

Depending on the nature of this application and your request, the information and documents requested from you must be sent to us completely and accurately. If the requested information and documents are not properly provided, there may be problems in carrying out the investigations into your request fully and thoroughly. In that case, we declare that we rightfully reserve our legal rights. For this reason, the form must be sent complete and must contain the requested information and documents, depending on the nature of your request.

INFORMATION IDENTIFYING THE APPLICANT AND INFORMATION ABOUT THE APPLICATION

You need to provide the following information so that we can identify the “applicant” of your application and carry out the necessary investigations depending on the nature of your request.

We ask for your contact details so that we can obtain more detailed information from you about your application, keep you informed about our review process and send you the outcome of your application.

To keep your personal data secure, we may contact you within seven (7) days of the date your request for information reaches us in order to confirm that you are the data subject, and we may ask you for certain information and documents for this purpose.

If any of the information and documents requested from you are missing, you will need to complete them and send them to us upon our request. Until the information and documents have been fully submitted to us, the thirty (30) day period for concluding the request set out in Article 13/2 of the Law will be suspended and will not run.

• I would like it to be sent to my address.
• I would like it to be sent to my e-mail or KEP (registered electronic mail) address. (If you choose e-mail, we will be able to respond more quickly.)
• I would like to collect it in person. ( Note: If it is collected by a proxy, a notarised power of attorney or letter of authorisation must be presented.)
